Program Overview
CAMH's Shared Care Clinical Outreach Service addresses the physical and mental health needs of homeless/under-housed people with serious and persistent mental illness who use hostels, shelters and drop-in centres in downtown Toronto.
The Shared Care team consists of registered nurses, outreach workers, visiting family physicians and a consulting psychiatrist. Using Recovery Model principles, the Shared Care team assists clients in accessing appropriate resources (mental health, physical health, financial, housing, recreational and vocational) and helps bridge clients to these services and other long-term supports to deal with their illness.
