This service provides coordinated and comprehensive treatment for people who have chronic schizophrenia or related disorders and live in the community.
Individuals with chronic schizophrenia or related disorders, with or without substance use, who are living in the community within the catchment areas.

Please ask your family doctor or psychiatrist to fax a completed CAMH referral form and Patient Screener Tool to Access CAMH at 416 979-6815. Each referral begins with an assessment to determine suitability for this service.
Individuals MUST reside within the catchment area.

Monday to Friday - 8:30 a.m. to 4:30 p.m. Extended Hours: We offer extended hours services that are clinic specific during the week and from 10 a.m. to 6 p.m. on holidays and weekends

Program Overview
These outpatient services (formerly known as CARE, Ventures and InterACT) provide a coordinated and comprehensive treatment program for people who have chronic schizophrenia or related disorders and are living in the community within the designated catchment area.
Catchment area: Humber River to the west, DVP to the east, Eglington Ave to the north and Lakeshore Blvd to the south.
Downtown Central Clinics
The approach is recovery-oriented and interdisciplinary. Holistic assessment; individual, family and group counselling; medication monitoring; case management; and peer support and education are also provided. ACT level and Intensive Case Management services are available.
Downtown East Clinics
The approach is recovery-oriented, interdisciplinary and holistic. Assessment; individual, family and group counselling; medication monitoring; case management; and peer support and education are also provided. ACT level and Intensive Case Management services are available.
Downtown West Clinics
The approach is recovery-oriented, interdisciplinary, and holistic. Assessment; individual, family and group counselling; medication monitoring; case management; and peer support and education are also provided.
