Psychosis Coordinated Care Service

Intended For

Patients who wish support in managing symptoms of psychosis in the high support context of a day hospital. Available to inpatients who would like more support on discharge to the community. Also available to current outpatients who would like more support in order to remain in the community or to prevent decompensation.

Additional Referral Details

Accepts referrals for CAMH patients only. Patients must have solid outpatient followup in the community in order to qualify. At the minimum, a well-established relationship with their family doctor who is willing to prescribe antipsychotic and other medication.

The Psychosis Coordinated Care Program offers an average of eight weeks service.

Referrals are triaged on acuity. Referring sources can expect contact within one business day from our intake coordinator.

Program Overview

CAMH's Psychosis Coordinated Care Program (formerly Partial Hospital Program) is designed to offer a highly structured treatment program for patients experiencing acute psychotic and severe mental health issues.

It offers intensive day hospitalization for patients, who may continue the stabilization process initiated on an inpatient unit. It also functions as a diversion from hospitalization for patients who require a highly structured treatment program to prevent decompensation and inpatient admission. The PCCS is composed of an interprofessional team of psychiatrists, nurses, social worker, occupational therapists, recreationists, peer support worker, pharmacist and psychologists. Patients may receive individualized, recovery focused, goal-oriented treatment to facilitate stabilization of symptoms in a less restrictive, non-institutional environment. The menu of services offered by the PHP include comprehensive psychological, medical, medication monitoring, social, functional, family assessments, therapeutic activity groups (including CBTp and Concurrent Disorder groups), social and vocational rehabilitation. PCCS supports a Tobacco Free environment.
